Prepare The Necessary Documents

You’ll need the following documents to avail of the educational assistance:

Photocopy of Government Issued ID

Before you become a beneficiary of educational assistance, you’ll need to show that you’re a resident of Parañaque. The ID should indicate the Parañaque address of the claimant and beneficiary.

Original copy of Barangay Indigency of Claimant

You can secure the certificate of indigency from your respective barangay. To know the requirements for this document, it’s recommended to make a personal visit to your barangay hall or call their office in advance. Ensure that the original signature is placed over the printed name of the Barangay Chairman or Barangay Kagawad. This is needed for DSWD requirements purposes.

Photocopy of Current School ID/Certificate of Non-Issuance of School ID signed by the School Registrar

To show proof that you’re a student, you need to submit a photocopy of your current school ID. If you don’t have an ID yet, you can ask for a certificate of non-issuance of a school ID from your educational institution. You can contact the school registrar for inquiries.

Certified True Copy of Registration with Assessment form

Visit your school registrar’s office and secure a certified true copy of your registration with the assessment form. This document should be signed over printed name by the school registrar.

Original Copy of Certificate of Enrollment

The last documentary requirement for educational assistance is the certificate of enrollment, which you can get from your school. The following must be indicated in this document:

  • Year and Semester
  • School Dry Seal
  • Signature over the printed name of Principal or Guidance Counselor
  • Signature over the printed name of Registrar

Fill Up the Transaction Form

After securing the documentary requirements, the last thing you’ll need is to fill up the transaction form. You can download the file from the Constituent Relations page, under the ‘Educational Assistance’ section.

After downloading the form, put a check on the type of request you’re availing of. In this case, mark the box next to the ‘Educational Assistance’. Then fill up all the blanks with your personal information. Ensure that all the information you put in is true and correct.
